Eligibility Requirements

1. Age Requirement: You must be 18 years or older.

2. Education Requirement: A valid GED or high school diploma is required.

Steps to Register

1. Fill Out the Registration Form:

   • Visit the registration form at: https://form.jotform.com/medcarepro4/registration-form-

   • Complete the form to the best of your knowledge.

   • Use your full legal name when filling out the form.

2. Pay the Deposit:

   • A $75.00 non-refundable deposit is required at the time of registration and is included in the form. (Important Update – Registration Deposit: Starting January 1, 2026, the registration deposit will increase from $50.00 to $75.00. This change is due to processing fees associated with registration.

Please note: The deposit remains non-refundable.

This policy applies to all new registrations after January 1, 2026.

This adjustment helps us manage cancellations and last-minute changes more effectively.

1. Review Class Dates:

   • To find the next available class for Medical Assistant or Phlebotomy, check the Upcoming Course Schedule tab on our website.

   • Note: All classes are held on-site. Online classes are only available for refresher courses.

2. Confirmation Email:

   • After completing the registration form, you will receive an email at the address provided. This email will include:

     • Course details

     • Start date, time, and location

     • A list of required books to purchase

     • Course policies and curriculum

Payment Information

1. Payment Schedule and Invoices:

   • You will receive a payment schedule and an updated invoice with each payment.

2. Card on File Requirement:

   • A card must be on file by your first payment to avoid late fees.

   • Late fees of $25.00 will apply for overdue payments.

Important Notes

• Students must attend on-site classes unless enrolled in a refresher course.

• Ensure all information entered on the registration form is accurate.

• Payments made are non-refundable.
